![]() Our Programs Toronto North Support Services provides two distinct but related services:
Community Support: ![]() The Community Support Program provides individualized community-based services to people with persistent mental illness, who are struggling to make a life for themselves in the community. The program serves over 300 individuals yearly. Toronto North uses an intensive case management model in which the Community Support Worker endeavors to provide assessment, service planning, skills teaching, linking to other services, crisis prevention and advocacy. We are a designated French language services provider, and staff members also speak a number of other languages including Hungarian, Farsi, Sinhalese, French and Hebrew. In addition to individual support, the program provides a variety of group experiences for its clients, both on-site and in the community, to assist them to meet their social support needs. The Supports to Housing component of the Community Support Program involves providing intensive housing support to 16 individuals with mental illness. This supportive housing initiative is designed to provide individuals with stability in the community by providing decent, affordable housing and on-site assistance.

Street Outreach: ![]() The Street Outreach Program is a mobile service that assists individuals who are homeless in Toronto. The service is provided five days per week, from 8:00 a.m. to 7 :00 p.m. Monday to Thursday, and 8:00 a.m. to 4:00 p.m. on Fridays. More than 400 different individuals are seen yearly, at an intensity and frequency that suits their needs. This varies from occasional contact to an intensive case management approach, depending on the client's willingness or ability to engage in a process of change. Regardless, every effort is made to see each client as a unique person who sets his/her own goals in partnership with the Street Outreach Team. Our primary objective is to help people to get off the street and access housing. This often involves addressing a variety of needs such as medical, vocational, social, legal and financial. Substance use and mental illness are additional challenges which some of our street outreach clients face. Connecting to a variety of resources and providing support once housed can be important ingredients to our clients' success. A specialized component of the program, Mental Health Outreach, provides intensive support to individuals who are homeless and are coping with a mental illness. Outreach staff who have expertise in mental illness provide a combination of outreach and long-term support activities.
