At Home / Chez soi

Between 2009 and 2013 Toronto North provided case management services on the Mental Health Commission of Canada’s 5 city research demonstration project called At Home/ Chez soi. In 2013 the Central Local Health Integration Network took over the funding for the Toronto programs, allowing them to continue to operate.  The current At Home program, a partnership with COTA and Across Boundaries, supports people who are homeless and have mental health issues using an evidence based Housing First approach. The three teams work together to provide either step-up or step –down service by transferring between teams when needed. The emphasis is placed on assisting clients to find housing, and then determining the supports they might need to be successful in the community.

Who is eligible for the service?

New clients are referred through The Access Point. Individuals must be homeless and require mental health supports to live successfully in the community.
