Your Right To Privacy

At Toronto North Support Services we take your privacy seriously. We are committed to ensuring that staff and clients understand the importance of privacy rights and obligations. The Personal Health Information Protection Act (PHIPA) balances an individual’s right to privacy with the need of health care providers to access and share this information. PHIPA gives a person the ability to control how his/her own personal health information is collected, used and disclosed.

What Information Do We Collect About You?

At Toronto North we collect personal health information about you directly from you or from the person acting on your behalf. The collection of this information is a necessary part of our program, and allows us to provide the best possible service. The personal health information we collect may include, for example, your name, date of birth, address, health history, assessments, and records of what happened during your visits with our staff. If we have your permission to do so, we will occasionally collect personal health information about you from other sources.

How Is Your Personal Health Information Used?

We use and disclose your personal health information to:

  • Provide you with quality care
  • Conduct quality improvement activities (such as client satisfaction surveys),
  • Plan, administer and manage our internal operations,
  • Provide training to staff and students,
  • Compile statistics,
  • Conduct risk management activities,
  • Comply with legal and regulatory requirements, and fulfill other purposes permitted or required
    by law.

How and Why do we Share your Information?

Toronto North Support Services will share your personal health information with outside organizations or individuals who are involved in your care only for the purposes of providing you with better service. You have the right to tell us not to do this if you choose. Please note that there may be times when we are legally required to share your personal health information without your consent. For more information on this, please speak to your worker or our Privacy Officer.

How Does Toronto North Protect Your Personal Health Information?

We take steps to protect your personal health information from theft, loss and unauthorized access, copying, modification, use, disclosure and disposal. We conduct audits and complete investigations to protect your privacy. We take steps to ensure that everyone who performs services for us protects your privacy and only uses your personal health information for the purposes you have consented to.

Your Choices

You may access, review and make additions to your personal health records. To withdraw your consent for any listed uses or disclosures, please speak to your worker or contact our Privacy Officer.
