The Access Point

The Access Point provides information and referral to community-based mental health, and supportive housing services in Toronto. Using a common application form, applicants can be referred to any of 50+ different service providers. The Access Point staff help applicants and referral sources navigate the mental health, and supportive housing system.

Which services does The Access Point provide access to?

The Access Point provides access to mental health Case Management, Assertive Community Treatment Teams (ACTT), Supportive Housing Services (SHS) and Early Psychosis Intervention (EPI) in Toronto.

Case Management is a service aimed at helping adults recover from mental health challenges. Typically this is accomplished by meeting with a mental health worker in a community setting who helps with setting goals, finding resources and navigating the various systems (health, income, housing, etc) in a person’s life.

Assertive Community Treatment Teams provide comprehensive, community-based treatment and support to persons with serious and persistent mental illness. They provide case management and psychiatric services, as well as assistance in such areas as employment, housing, and recovery from substance use.

Supportive Housing is for individuals with mental health and/or addiction challenges who require, and are willing to accept, some level of support within their housing situation. All housing accessible from The Access Point is affordable through rent-geared to income or rent supplements.

Who is eligible for these services through Access Point?

These services are available to adults 16 years of age and older who are experiencing mental health problems that are seriously affecting their lives.
Our catchment area is the City of Toronto. Referrals are accepted from service providers, family and friends and those looking for services for themselves.
